Request for Permanent Clarification of Namesake Record

Dear Sir/Madam,

I am writing respectfully to request for your kind assistance regarding the recurring issue of namesake records under my name. Each year, when I apply for the renewal of my visa/clearance, I am informed that there is a “hit” in the system because another person with the same name has a criminal record.

I wish to respectfully state that:

I am not the same personas the individual with the criminal record.

Every year, I am required to undergo the same clearance process and pay additional fees only to prove again that I am not related to this case.

This situation causes unnecessary financial and administrative burden on me, even though I have no involvement in any criminal activity.

In this regard, I respectfully request:

That an annotation or permanent remarkbe made in your records to clearly indicate that I am not the same individualas the person with a criminal record.

That future applications under my name be processed without requiring me to repeatedly spend money and time to prove my innocence.

If possible, that my Alien Certificate of Registration (ACR I-Card) or passport number be officially linked to my record, in order to avoid confusion.

I am more than willing to provide any necessary supporting documents, including my passport, ACR I-Card, or previous NBI clearances, to facilitate this request.

Thank you very much for your kind attention to this matter. I sincerely hope for a fair and practical resolution, so that I may continue to comply with the laws of the Philippines without unnecessary hardship.

关于重名导致重复证明与收费的澄清与请求

尊敬的局长：

您好！本人谨此致函，恳请贵局协助处理本人在申请签证/无犯罪证明过程中长期遇到的重名问题。

在每年的签证更新或申请无犯罪证明过程中，系统均显示与本人同名的他人存在犯罪记录（“hit”），因此本人被要求再次提交证明文件并支付相关费用。

在此，我郑重说明：

本人并非该涉案人员，与该犯罪记录无任何关联。

每年重复进行同样的手续，仅为证明“重名者并非本人”，对我造成了额外的经济负担与不便。

这种情况使守法的外国居民承受了本不应承担的责任和费用。

为此，本人谨向贵局提出如下请求：

请在贵局系统中做出永久性备注或注释，明确记录“本人并非同名涉案人员”；

允许将本人护照号码或外国人登记证（ACR I-Card）与系统记录绑定，以避免未来继续发生误判；

在未来的签证或相关手续办理中，免除我因重名而重复支付不必要的费用和手续的困扰。

本人愿意随时提供相关文件，例如护照、外国人登记证（ACR I-Card）、以往的 NBI 无犯罪证明等，以便贵局核实。

衷心感谢贵局的理解与协助，并期盼得到一个公平合理的解决方案，使本人能够依法合规地在菲律宾居住生活，而不必承受重复性、不必要的行政负担。
